capped but being over charged

Anyone help I have a £5 cap on my phone but have been over charged small amounts on occasional bills adding up to a total of £18!

I have checked and In still inside my allowances and do mot inderstand why you have charged me £10 extra. I havr alwsys been under my capped amount.
