I need help with cancelling my contract? I'm unable to login either?

I have broken my phone and forgotten my pin to allow it ti work in a new phone and have forgotten the password and username and i want to cancel my contract is there anything i can do

I wish to cancel my contract and pay it up but my phone doesn’t work at all and never has done

This phone was an upgrade last January and it worked for one day and never worked again.
