I've ordered a new phone and sim from a different provider, as my contract with ID Mobile ends in a couple of weeks.
The new phone should arrive in 2 - 3 weeks I have been told.
When should I request my PAC codes from ID? When the new phone arrives or before that?
Do I need to do anything else to cancel my contract, other than get my PAC code?

As PAC codes last for 30-days, you can effectively request one now if you are going to change providers within the next 30-days. After 30-days, a new PAC code can be issued anyway.
Once the PAC code has been used by your new provider, the plan will automatically cancel on our system and a final bill will be issued.
