fraudulant account setup

There have been two fraudulent accounts setup using my name and old address. 

these need to be closed. This is the second time iD mobile has allowed fraudulent accounts to be setup with theft of my identity. 

can someone please contact me to explain how to close these accounts and associated debt recovery collections.
