View originalPorting number to ID but to a new phone that uses a different sized sim card

I want to port my old number to ID from another network. My old number is on an old Nokia A300 which uses a full sized sim. I want to port my old number to my new ID account to use on a new Samsung J3 which uses a smaller sim (and has a different current number which was allocated when I bought the phone last week from Carphone Warehouse). How do I move my old number from my old provider (currently on an old phone and big sim), to a new provider (ID) and a new phone which uses a small sim? I cannot transfer the old sim from my Nokia to my new Samsung because it won't fit.
Hi Cheshire Cat.
First, you'll need to contact your previous provider to request a PAC
Once you have this, you'll be able to use the iD app or website, my.idmobile.co.uk to give us the PAC which will transfer your existing number to us.
You can find out more information here or let us know if you need more help.
