newbie trying to port in a number

hi on filling in pac code and detail s to move my number to id. my a/c says it will be ported on Friday 28 May and a text message says there is an issue in that they have got either

1   I'd mobile number

2 last name 

3 transaction number.              are incorrect  contact customer services? HOW. ? I AM FINDING THIS UNBELIEVABLE DIFFICULT.  CAN ANY BODY HELP?   AS  TIME IS RUNNING OUT JMAX
