Reporting a stolen phone


My phone was stolen 6 weeks ago. I have barred the sim and got a replacement sim activated in my new phone. I spoke to the police about blacklisting my old phone and I was told that my tariff provider has to do this. As there is no ID mobile number to call to0 get help, I have struggled to work out how to do this immediately. Could you please help in blacklisting the physical phone (not the sim as it is now transferred to my new sim)? Although with the length of time my phone has been probably sold sadly :(
