Cancelling an old contract

A phone number/account that I USED to have is regrettably now draining my account. I have lost the original number and any of the details but it still comes up on my bank account, taking over £15 from me. While I am satisfied with my current phone and bill, the other one is one I would like to try and cancel to avoid further monetary losses. Thank you
