Feedback box does not appear after live chat session

Hi, I have used live chat on 2 occasions and each time have been asked if I would fill in the feedback survey after closing the chat box which I agreed to do however each time I close the chat box there is no feedback form to fill in.

Is there another area of the site in which I can provide this feedback? The people I spoke to were very helpful and I'd have liked to have given them high ratings!

No big deal, but would have been nice to do that for them! If Zizipho and Landiswa are reading this, thank you!
