i have lost my phone insurance asking Proof of Usage (mobiles / SIM devices only)

I have lost phone few days ago and i now my insurance asking me. 

following evidence.

proof of blacklisting mobile sim devices only, written confirmation.

since ID does not have someone to talk on a phone, i have blocked the sim online and also reported automated service.

Proof of usage mobile sim devices only, proof of usage include device make, model and IMEI Number

and the first and last date device used.

can someone help me please?
