my phone is asking for a puk code??

Hi. My phone is asking for a puk code. I did take the sim out of the phone to use it in a different handset but it asked for a puk code to use it. I’ve had it back in the original handset for a couple of weeks no problem but now it’s asking for a puk code again and I can’t use it. Why is this? I have once before changed handset for a week and it was fine. How can I get a code please?
