PAC code not working

I am trying to port my number to iD, but I keep getting an error message when trying to submit the form. 
All of my information is correct.

 

This is the message:

 

We've tried to process your PAC, but there is an issue with the information you've submitted. Please check that you have provided the correct details and try again.“

Okay @Lucy p, when you can’t submit your old provider’s PAC online, you can give your PAC to iD by getting in-touch using one of the options below:

  1. Send iD a private message on facebook (using Messenger).
  2. Send a DM (direct message) to iD on Twitter.
  3. Chat online at https://idmobile.co.uk/live-chat - the iD agents work 9am - 8pm weekdays (and 8am - 6pm at weekends).

Out-of-hours messages on facebook, or Twitter, are picked-up when the social media team next start work.
