Sim Card number- insurance claim

Hi Team,

i lost my phone and sim card 2 months ago in August. I then got a new SIM from currys.

my insurer needs to see  the first and last dates the device was used with OLD SIM card, along with the IMEI number and/or serial number.

How can i get the details of my old sim card number?
