Help with complaints procedure

I want to make a complaint to ID mobile but am unable to do so as I do not have the mobile phone number which is required for the complaints form and for a phone call. I do not have the number as the account was set up fraudulently with my bank details.

Hello. A phone contract was taken out with you without my knowledge or consent using my bank details. I do not have the phone number or any access to the account. I have had no communication from ID mobile but have now been contacted by debt collectors. Please can you advise me of the best course of action. I have a crime number. Thanks
