Data roamimg on countries not losted on add on

I have been charged nearly £60 for absolutely nothing I’m Brasil. 
 

I’m outside the add on destinations and I’m not using any of the services of my phone. data or calls as none work. I have disabled the data roaming and also there is a cap on my account of £45. In summary my phone doesn’t work here. 
 

However, I have been charged £57 pounds for absolutely nothing. Mobile data is off and data roaming was capped and still is charging me for no service. I want to dispute this charge, this is a scam that not going to fall. 
 

How can I report and dispute this charge? This is unacceptable.
