EU ROAMING CHARGED WHEN SHOULD HAVE BEEN INCLUDEF REFUND PLEASE

Hi how can I get a refund on an eu roaming charge added which should have been free as in Spain. Also the spending went over my capped spending which should not happen? Thanks
