Blacklisted question

Hello,

I lost my phone earlier so I rang ID up to explain and they have locked the sim so it cannot be used and have blacklisted the model for security. I am making a claim through my insurer and they require the following information

Proof of barring

Proof of sim card replacement

Proof of purchase (that is obvious one where to find that)

Where can I find the first 2 please?
