My minimum contract period is ending on 14th December. Through the app, I have ordered a new phone, and when ordering was prompted to enter a new direct debit. How do I know my existing contract will transfer to this new plan?

If you selected it through the Upgrade section it will do it automatically. If you check your App when the new phone arrives, you’ll see your allowances will update to reflect the new agreement, that’s a good way of knowing for sure.
