Closing account after sim card has been thrown away

My daughter stopped using her ID account (in my name) a few months ago, and threw the Sim card away before I had chance to cancel it. how do I close the account as the DD keeps coming out each month.  I can no longer log into that particular ID account online either as when i request a password it sends it to the phone number where we no longer have the sim.  Please help. Thanks
