moving sims with a PAC code

Hi new customer here. 
just got my new esim today and also have my pac code to transfer my number. 
when I enter the details into the form found in these guides I get an error message  

see below

i have entered to info several times I didn’t enter the code when I placed my order.

any ideas

    To clarify, are you moving your number over to us from another network?

     

    Are you using the transaction code beginning with a 4 and not a 1 to confirm?
