my phone has been stolen and my insurance company

Hi,

my phone has been stolen and my insurance company requires the following:

  • Written confirmation that the device has been blacklisted, including the IMEI number

  • Proof of usage showing the device make, model, IMEI, and the first and last dates it was used

Could you please provide these documents at your earliest convenience?
