Proof of Usage

I need a proof of usage for my insurance claim. I spoke with a live agent who said this will be on the proof of purchase document.

I have received the proof of purchase document but there is no proof of usage on there.  

Can somebody send me proof of usage please. Been trying for 2 hours to get through on live chat but theres no agents available.
