Proof of Usage Document

I have request a Proof of Usage document from an live agent, but they said they cant provide the IMEI number or anything that can uniquely identify the phone that I have been using. The whole purpose of a Proof of Usage document is to show exactly what phone was in use during certain times, and I need this document to show to my insurance to make a claim.

 

Specifically: “Proof of usage should include the device make, model, IMEI number and the first and last date the device was used”. How do I obtain this?
