Proof of usage for insurance


I need proof of usage for an insurance claim for my motorola edge 50 pro device, which was not purchased directly from you with my contract.

I told am struggling to find the above. 

My insurers are asking for proof of purchase (I have the receipt) proff of usage ( I have a bill ) proof of when SIM activated and if still activated. This I can't prove 

 

It's a damage repair claim for smashed screen.
