Struggling to transfer old number to new phone

I have tried to do everything asked to transfer my old number from o2 to iD and every time I try it says I have entered details incorrectly even though I haven't.

I got the PAC number and entered all the other relevant details but keep getting the same error.  How do I resolve this?
