Upgrade free gifts havent shown up

I have recently upgraded my phone and because I'd found a much better deal elsewhere, when I spoke to someone on the upgrade team I was offered a free charger and £100 cashback with my upgrade which I accepted over the phone.

I have had my new handset delivered but there was no charger as promised and there doesn't appear to be any sign of the cashback either. How do I claim these free gifts?
