Pac Code change not working

i want to keep my old number from my old provider but the page that i enter my details on 

leaves a message 

“We've tried to process your PAC, but there is an issue with the information you've submitted. Please check that you have provided the correct details and try again”.


i have entered everything correctly and got my pac code from Vodaphone

can someone please help me as ive tried many times and the above message appears
