PAC switch is failing

PAC switching error

I’ve entered all the details on the switching page (copied and pasted so no risk of any typos)

I get the error “We’ve tried to process your PAC, but there is an issue with the information you’ve submitted”

I got a second PAC from my old provider and tried that but still getting the same error

please help

I’ve tried switching via the website and the iD mobile app - same error
