How do I add replacement sim to new account when I don't have ID tel number?

I purchased sim online, but didn't receive the sim. I couldn't figure out how to get a replacement so went to curry's and got a new sim there. 

 

Now I have registered on the ID app, but can't figure out how to get a tel number or attach it to my account? 

 

Please help.
