Problem with transferring my old phone number

SOLVED NOW.I have followed the steps indicated in the website but I get the following error message 

We've tried to process your PAC, but there is an issue with the information you've submitted. Please check that you have provided the correct details and try again.

The information that I have entered is correct (as far as I know) so not sure what the issue is. Any help would be appreciated. Thanks.
