Where can I find proof of purchase for my new upgrade where it clearly states what make and model of phone i have got and how much my monthly plan is? I need this so I can claim a chromebook and cant seem to find it anywhere

Upon request, the initials and county of the winner will be available by sending an SAE to the Promoter at the address below. Such requests should not be sent until 28 days after the draw.
