Proof of usage & blacklisting


I have had my phone stolen and my insursnce company have requested: 

Proof of usage which will need to confirm the IMEI number of my device, as well as the first and last usage dates. 

Proof of barring which will need to confirm the IMEI number of my device and the date that my IMEI number was blacklisted.

I have requested this via live chat but have been provided with a copy of my bill and proof of purchase of my sim which is not what I have requested.

Is anybody able to confirm how I go about obtaining these documents?
