Failed credit check due to two accounts maybe?

I am currently with ID mobile on a monthly rolling contract. Wanted a new phone upgrade so found a deal with ID mobile but going through carphone warehouse. Failed the credit check the first time which I think was due to me misspelling my name ‘I put Naomo when it should be Naomi) tried again and it still failed. 
 

could this be due to me already having an account with ID mobile?
