Issues with PAC and registering account

Ordered new SIM, have PAC code for my old O2 number and have input transaction number (yes, it begins with a 4) and yet it keeps coming up with an error message that ID are “unable to process PAC as information is incorrect” 

the information is correct.

I tried registering my account on the app using the provided ID mobile number but then that says details do not match records. Could someone please assist?
