consolidate multiple accounts

Hi,

I have 5 mobile accounts with ID mobile for myself and my 4 children.  I want them consolidated under one single account.  I lost the login details of 4 of the accounts but have the mobile numbers and the bank account that they collect payment from.  Please advise how to proceed.
