Keeping my mobile number

I have recently changed to a different network. I want to keep my mobile number and was given the PAC code recently and yet it doesn't work and was told to request another code as it had expired. I am well within the time period given before I would lose the opportunity to keep my number.

How do I get a new PAC code?
